It all starts with a friendly chat — by phone, email, or through our contact form.

We’ll ask a few general questions like:

  • Who needs support (you or a loved one)?
  • What kind of care is being considered (hourly, daily, post-hospital, etc.)?
  • Are there any specific needs or concerns (mobility, memory, medication)?
  • Your location and preferred times for visits

This helps us prepare the right person to carry out the assessment — and ensures we’re a good fit from the beginning.

🏡 Step 2: In-Home or Virtual Assessment

We’ll arrange a convenient time for one of our experienced care coordinators to meet with you. This can be done:

  • In your home
  • In hospital (if you’re being discharged)
  • Virtually (via phone or video call)

During the visit, we’ll take time to understand:

  • Your daily routine and lifestyle
  • Health conditions or medical needs
  • Mobility, nutrition, and medication requirements
  • Any personal care or emotional support needed
  • Safety around the home
  • Preferences about carers (language, gender, times of day)

We’ll also answer your questions, listen to your concerns, and ensure everything is done at your pace.


📋 Step 3: Creating Your Personalised Care Plan

Using everything we’ve learned, we build a clear, tailored care plan that reflects:

  • What support is needed and when
  • Specific tasks or routines to follow
  • Emergency contacts and preferences
  • Notes on communication style, mobility, and wellbeing
  • Any equipment or adjustments required

This plan is shared with you and your family (if you wish), and is reviewed regularly to keep up with changing needs.


🤝 Step 4: Matching You With the Right Carer(s)

We don’t just assign anyone — we match you with a small team of carers who are:

  • Experienced with your specific needs
  • Local and available at your preferred times
  • Chosen based on your personality and preferences

You’ll always know who’s visiting, and we’ll introduce your carer(s) before care begins.


🛠️ Step 5: Starting Care, Your Way

Once you’re happy with everything, we confirm your start date and begin care.

You’ll receive:

  • A clear schedule of visits
  • A contact number for your local office
  • Ongoing communication and check-ins
  • Access to digital care notes (if you choose)
