Arranging care for yourself or someone you love can feel like a big step. You may have lots of questions about what happens during an assessment, what you’ll be asked and what happens afterwards.
At Fuchsia Homecare, we want to make the process as simple and comfortable as possible.
A care assessment is not about ticking boxes or making decisions for you. It’s an opportunity for us to get to know you, understand what support you need and talk through how we can help you stay comfortable, independent and in control at home.
No pressure. No complicated jargon. Just a conversation about you and the support you need.
Here’s what you can expect.

The first step is simply getting in touch with us.
You can speak to our team by phone, email or through our contact form. We’ll ask some basic questions to understand your situation and what you’re looking for.
For example, we may ask:
This first conversation gives us a better idea of how we can help and allows us to arrange an assessment at a time that suits you.
There’s no pressure to make a decision. We’re happy to answer your questions and talk things through, even if you’re still exploring your options.

Once we’ve had our initial conversation, we’ll arrange a care assessment.
Depending on your circumstances, this may take place in your home, in hospital or virtually.
This is where we take the time to understand what everyday life looks like for you.
We’ll talk about things such as:
We also want to hear what matters to you.
Perhaps you want to remain independent with certain tasks, continue going out regularly, have someone to chat to during the day or simply have a little extra help around the home.
These details are important because good home care should fit around your life — not completely change it.

After the assessment, we use everything we’ve discussed to create a personalised care plan.
Your plan will set out the support you need and how you would like your care to be provided. It may include:
We’ll talk through your care plan with you so you understand what has been agreed.
And importantly, your care plan isn’t set in stone. As your needs change, we can review and update your support so it continues to work for you.

Finding the right people to provide your care is an important part of the process.
We look at your care needs, routines, preferences and personality when putting your care team together.
For example, we consider:
Our aim is to provide consistency and familiarity, so you can get to know the people who visit you and feel comfortable with your care team.
We’ll also let you know who will be visiting and make sure everything is clear before your care starts.

Once everything has been agreed and you’re happy with your care plan, we’ll arrange your start date and your visits can begin.
We’ll make sure you have the information you need, including your care schedule and how to contact our team if you have any questions.
From there, we continue to stay in touch and review your care to make sure everything is working well.
If something changes, you can always speak to us. Your care can be reviewed and adjusted as your needs, routines or preferences change.
Most importantly, you should feel listened to.
You don’t need to have everything worked out before speaking to us. You may not even know exactly what type of care you need yet — and that’s completely fine.
Our job is to listen, ask the right questions and help you understand your options.
A home care assessment is simply the starting point for creating support that is personal to you, your routine and your life.
If you’re considering home care for yourself or someone you care about, we’re here to help.
Get in touch with Fuchsia Homecare to arrange a friendly, no-pressure conversation about your care needs.
You can ask questions, talk through your options and take things at your own pace.
